Accident Prevention Plan
EM 385-1-1, 2024 Edition (15 March 2024) · ENG Form 6293
Learn to prepare an Accident Prevention Plan that a USACE or NAVFAC project will accept — the work, the site-specific plans it requires, the people, the safety program and the appendices — and prove it by completing an APP worksheet that gets read and graded by the instructor.
Why this course exists
No accepted APP, no work.
EM 385-1-1, paragraph 2-7.b: “No contract work can begin without a KO or COR accepted APP.”
The worksheet is mandatory
EM 385-1-1 (15 March 2024) requires ENG Form 6293 — thirteen parts and six appendices. A reviewer reads it part by part, and every part has to say something about this project.
Part 11 is where APPs fail
Sixty-six rows of site-specific plans, each needing an option and a justification. “Not applicable” down the side of the page tells the reviewer nothing.
The work drives the plan
Every plan attached and every person named should trace back to a line in the work inventory. Write the APP from the scope of work outward, not from Part 1 downward.
The assignment is your own project. You complete the APP worksheet for a project you have worked on or are working on now, so you finish the course with a worksheet you can actually use.
Who it is for
Anyone who prepares, approves or maintains an APP
No prior EM 385-1-1 experience is assumed. The second module teaches you to navigate the manual from scratch.
Project Managers
Who prepare the APP and submit it for acceptance.
Superintendents
Who run the work the APP describes.
SSHOs
Who keep the APP current for the life of the project.
QC & Safety Officials
Who review and approve it before it goes to the Government.
Learning outcomes
What you will be able to do
Upon successful completion of this course, participants will be able to:
- Explain EM 385-1-1 (2024 Edition) requirements for preparing, approving, and accepting an Accident Prevention Plan (APP).
- Locate the plan requirements that apply to a work activity by navigating to paragraph 7 of the relevant EM 385-1-1 chapter.
- Classify a described workplace incident by its injury-source code and hazard family.
- Select control measures for a hazard using the hierarchy of controls, in order of decreasing effectiveness.
- Describe a project’s work: its phases, definable features of work, equipment and high-risk activities.
- Select the site-specific safety plans the project needs, and justify the ones it does not.
- Identify the safety personnel an APP must name (the Site Safety and Health Officer, Competent Persons and Qualified Persons) and the documents that prove their qualifications.
- Complete the safety program sections of the APP: policy, inspections, accident reporting, oversight, severe weather and site coordination.
- Assemble the worksheet and its appendices into an APP ready for the contractor to approve and the Contracting Officer (KO) or Contracting Officer’s Representative (COR) to accept.
- State when an accepted APP must be updated, who updates it, and where it must be kept on site.
- Prepare an Accident Prevention Plan Worksheet on ENG Form 6293 for a project.
Curriculum
Five modules, in order
Each instructional module ends with a ten-question quiz. The assignment is graded by the instructor.
Course Orientation
What the course covers and what it asks of you, how the platform works, what to download, how the pop-up checkpoints and module quizzes behave, and what you have to finish to earn the certificate.
Navigating EM 385-1-1, 2024 Edition
All thirty-seven chapters run the same ten numbered sections, so learning one chapter teaches you all of them. Paragraph 7 of every chapter is Minimum Plan Requirements — the paragraph every row of Part 11 points to.
Workplace Hazard Identification and Mitigation
The five core hazard families, the hierarchy of controls in order of decreasing effectiveness, and the twelve injury-source codes — Struck By, Caught Between, Fall to Below and the rest — with a worked example for each.
Preparing an APP — The Six-Step Method
Define the work, select the plans, name the people, set the program, assemble and approve, keep it current. Each step is taught, then applied to one project: My Repair Garage, a 4,000 square foot concrete masonry maintenance facility, shown part by part on the actual ENG Form 6293.
Part 11 is worked row by row — Option 1, 2 or 3 — with every selected plan traced back to the work that requires it.
The APP Assignment
You complete ENG Form 6293 — all thirteen parts — for a project you know, using the six-step method. You mark in Part 13 which appendices your APP would carry; you do not have to write the plans themselves.
It is read against eight checks and returned within two business days. There is no percentage — it either meets the standard or it comes back with the reasons, and you revise and re-upload.
Assessment
How you are assessed
Three layers. Each one establishes something the others do not.
| Layer | What it is | To pass |
|---|---|---|
| Module quizzes | Ten multiple-choice questions at the end of each instructional module, one correct answer each. Every learner answers all ten. | 70% — seven of ten. Two attempts, then the module resets and you work through it again. |
| The APP worksheet you prepare | ENG Form 6293 for your own project, read by the instructor against eight checks. | Pass or revise. Two business days’ turnaround, up to three rounds. |
| Final examination | Ten multiple-choice questions drawn at random from the module question pool. Twenty-minute limit. | 70%. Two attempts. |
There are also short pop-up checkpoints inside each video. If a checkpoint is not answered within a minute the video returns to the last bookmark, so the course only advances when someone is actually watching.
To finish the course you pass every module quiz, submit an acceptable APP worksheet, pass the final examination, complete the course evaluation and the attestation statement, and then download your Certificate of Completion.

Do I have to write all the site-specific plans?
No. The assignment is the APP worksheet, ENG Form 6293. You select the plans your project needs in Part 11 and mark in Part 13 which appendices your APP would carry. The complete My Repair Garage APP shows you what the plans themselves look like.
What if my APP worksheet comes back for revision?
You receive an email naming exactly what needs to change. You revise and re-upload. Up to three rounds are available.
What happens if I fail a module quiz?
You have two attempts. If you do not pass in two, your progress resets to the start of that module and the quiz reopens once you have worked through it again. Resets happen within two hours on a weekday between 8:00 AM and 4:00 PM Pacific time, and otherwise the next morning between 8:00 and 10:00 AM.
Can I buy several seats for my team?
